New Medical and Dental
ID Cards are in the
Mail to all Members
Every member in the medical and/or dental plans will be getting new ID cards. One card each for Medical and Dental.
Your Medical card should list the member and each dependent and show coverage as Med/Vis. Your new Dental card will only show the Member name. The Dental provider will confirm dependent participation when the card is presented.
Changes made to the new medical cards include a new color for 2017. Please make sure that after December 31 you use your new tan colored medical, vision and pharmacy card and throw out the old Medical card that had dental on them.
A new separate dental card will be issued from United Health Care. The new UHC dental plan will start January 1, 2017. We will no longer be affiliated with Dentemax.
Should you need more than the two cards that are mailed to you, please log into your account and request a new card. They are issued in sets of two (2).

Generics reduced to $15 30 day retail, $30 for 90 day supply mail order

Formulary Drugs 10% cost share, $50 minimum-$100 maximum (30 day retail)

Non-Formulary Drugs 10% cost share $100 minimum-$200 Maximum (30 day retail)

Coordination of Benefits for individuals that have more than one medical insurance policy, changes from 100% coverage to same as normal Plan design. Medicare excluded.

Out-of-Network Deductible increased to $6,000 individual $14,000 family.
Dental Plan
The Board of Trustees has decided to change the current Dental Plan to a United Health Care Dual Option PPO/HMO Dental Plan in order to give the members choices for their dental insurance coverage.
Everyone will still have the opportunity to continue with the same dental coverage (similar but different network) as we currently offer, but you will also have the choice to elect a less expensive dental insurance plan option if that works better for you and your family.
Both options will be offered by United Health Care and the Benefits Fund will no longer offer the self-insured plan through Dentemax. It is advised to review the list of participating dentist to confirm that you current providers are in the new network.
Members living outside the state of Florida will only have the PPO option.
